Your clinical team works alongside you to build a discharge planning process that runs alongside your treatment. This includes a step-down pathway in levels of care to make sure you know where to turn for ongoing care and support.

    Our Aftercare Planning Approach at AMFM

    Aftercare planning at AMFM is a structured, organization-wide process throughout your treatment. Every one of our case managers submits a formal aftercare request within your first week of admission. This request covers everything from diagnosis and levels of care needed to your housing situation, insurance situation, and expected discharge date. 

    The process involves collaboration across every level of your care, evaluating whether a step-down into Mission Connection – AMFM’s own outpatient program – would be a good clinical fit. If so, our finance team will work with you through the practical side of things, such as your insurance benefits, and an intake appointment will be set prior to discharge. 

    Should Mission Connection not be a good fit, then our team will identify and research additional options, ensuring everyone leaves with a realistic and considered aftercare plan. 

    What to Expect in Aftercare Planning at AMFM

    The aftercare planning process is always collaborative and inclusive at A Mission For Michael. Here’s a breakdown of what you can expect: 

    Planning Beginning at Admission

    Within your first seven days with us, your case manager will complete a formal request for aftercare services, so planning can begin well in advance of graduating from inpatient treatment. This request will include any pertinent information about your situation and treatment.

    Assessing Your Options

    Your case manager will work hand-in-hand with the rest of our team to determine the next best steps for your care. When possible, this includes transitioning into Mission Connection, AMFM’s sister outpatient program, which gives you the best continuity of care within the same system. 

    Sorting Out the Details

    If Mission Connection is the right fit for you, then our team will check your benefits and establish an intake appointment and a meet-and-greet before discharge to ensure a smooth hand-off. 

    Building Your Continuing Care Plan

    During your stay, AMFM will engage you with our eight-week Continued Care Journey curriculum, which works to equip you with the knowledge and practical tools to navigate life after treatment.

    The Continued Care Journey Curriculum

    Aftercare preparation at A Mission For Michael includes the aforementioned Continued Care Journey – an eight-week module built directly into your treatment plan. The curriculum covers the full continuum of mental health care, helping you understand exactly what each level of care entails: partial hospitalization programs (PHP), intensive outpatient programs (IOP), and standard outpatient

    We’ll cover the rationale and best practices for stepping down your care and what to expect from the transition. We also talk you through what to be aware of after treatment has concluded and how to keep your recovery momentum going. For instance, this might include:

    • Navigating FMLA and disability paperwork, if necessary, to better understand your rights and how to manage documentation when returning to work
    • Building your natural support system to identify and strengthen connections with others in your community who can play an important role in your long-term healing
    • Exploring peer and community groups to engage with 12-step programs and other peer-led support networks to complement your ongoing care
    • Vocational skills and practical preparation for re-entering the workforce, including resume writing and interviewing skills for those who are ready to take the next step

    For many leaving our residential program, Mission Connection offers both in-person and virtual care within the AMFM family, making your step-down process as seamless as possible. 

    Mission Connection’s continuing care for mental health is delivered via a wide range of evidence-based treatment approaches. These include CBT, DBT, EMDR, ACT, and psychodynamic therapy, alongside trauma-focused treatment and somatic approaches. 

    Both PHP and IOP are available, giving you the comprehensive support you need at an intensity that matches where you are. These approaches can allow you to spend more time at home, with the flexibility to step down gradually as your confidence and abilities grow.[2] 

    If Mission Connection isn’t the right fit for you, our team will work to ensure your step-down plan includes everything you need for continued success. For example, we can provide vetted referrals to other providers or your own personal recovery plan.
