We remain committed to data-driven quality improvement through the consistent use of validated intake and discharge assessments. In addition to measuring key clinical indicators such as depression, anxiety, suicidal ideation, and self-harm behaviors using the PHQ-9 and GAD-7, we expanded our outcomes reporting in 2025 to include the Flourishing Scale, providing a more comprehensive view of each client’s overall well-being. By tracking both symptom reduction and broader measures of psychological health, we are able to better evaluate the effectiveness of our care and ensure our interventions remain responsive, personalized, and clinically sound.
The PHQ-9 and GAD-7 are widely used, evidence-based screening tools designed to assess the severity of depression and generalized anxiety. Together, they support accurate diagnosis, inform treatment planning, and allow us to monitor progress over time with consistency and reliability.
All outcomes data is collected through our OutcomeTools portal, integrated with our Electronic Health Record system, with input from both clients and clinical staff. Assessments are completed at intake, throughout treatment, and at discharge to track progress. Our data collection processes are guided by rigorous standards to ensure accuracy, consistency, and the integrity of our reporting. Additionally, ongoing client feedback is gathered through regular satisfaction surveys to ensure we are continually improving the experience and quality of care we provide.
